Shipping

To ensure successful completion of your case, please complete the following steps:
*Not For Clients In New York or New Jersey

  1. Carefully fill out your Rx, being sure to include Dr. Name, Patient Name, Shade, Tooth # and Type of Restoration Material.
  2. Carefully package your case, including the Rx, and tape box securely closed. Next, place casebox into FedEx envelope or box. Seal envelope and apply FedEx airbill. Keep your tracking number.
  3. Call Customer Service at 1-800-771-9840 to schedule your case pick-up.
  4. Track the shipping status of your account at www.fedex.com or by calling Elite Dental Customer Service at 1-800-771-9840.
  5. Your case will be returned via overnight courier service in just 10 working days. Working days do not include weekends or holidays.

*For Clients in New York and New Jersey

  1. Carefully fill out your Rx, being sure to include Dr. Name, Patient Name, Shade, Tooth # and Type of Restoration Material.
  2. Carefully place your Package in the Lab bag with the prescription and the name and address of your office and where it is going.
  3. Call the lab to schedule a pick-up and if it is before 2pm our courier service will pick it up the same day.
